When a user he doesn't have an item but just an item (the crate in this situation) comments on a profile, the shadow pops up in the comments instead of the blank spot of the ago.
I don't know if this is intentional or not!

@Baz: Ah yes sorry! It was adorable Ms Kitty on my profile! https://www.voltraonline.com/voltie/cedes Clicking on her profile or seeing her in the forum proves she's equipped just the crate with no avater base! I've seen the crate avi since yesterday I believe!

Yeah, its showing that way on my side as well. On the top where it says "Hi Miss kitty" where my avatar should be, its just a shadow. I think the logic behind it is because there is no skin, it shows the shadow, despite having something equipped.
