It's a movie about a man who's face is disfigured so that he always looks like that, and falls in love with a blind woman. But that smile is the inspiration of the Joker's wide grin.
Well it's based on a book written by Victor Hugo, the man who created The Hunchback of Notre Dame. He had a thing for ugly characters.
